Unilateral surgery may be more effective than bilateral surgery for correcting intermittent exotropia, but there is no evidence on the optimal timing or indications of surgery.
A Cochrane review included 1 study with 36 subjects. Bilateral surgery resulted in satisfactory results in 10/19 (52%); unilateral surgery 14/17 (82%), p<0.02.
Comment: The quality of evidence is downgraded by imprecise results (few patients), by limitations in study quality and by indirectness (clinically important questions not answered).
