Smoking cessation medication for preventing weight gain after cessation

Therapy with nicotine replacement, bupropion, or varenicline, appears to have some effect for preventing weight gain after smoking cessation in short term.

A Cochrane review consisted of interventions for preventing weight gain after smoking cessation. Nicotine replacement therapy (NRT) resulted in attenuation of post-cessation weight gain (mean difference in weight change (MDW) -0.69kg, 95% CI -0.88 to -0.51, 19 studies, n=2600 at the end of treatment, and -0.42kg, 95% CI -0.92 to 0.08, 15 studies, n=1334 at 12 months), with no evidence that the effect differed for different forms of NRT. Bupropion 300mg/day vs placebo limited post-cessation weight gain (MDW at the end of treatment -1.12 kg 95% CI-1.47 to -0.77, 7 studies, n=869, and at 12 months -0.38kg, 95% CI -2.00kg to 1.24kg, 4 studies, n=252). The weight reducing effect of bupropion was not dose-dependent (300mg vs 150mg or 100 mg). Varenicline 2mg vs placebo limited post-cessation weight gain (MDW at the end of the treatment -0.41 kg, 95% CI --0.63 to -0.19 kg; 11 studies, n=2008, and at 12 months 1.11 kg, 95% CI -0.75 to 2.98 kg) and no follow-up data were currently available. In three studies, participants taking bupropion gained less weight at the end of treatment than those on varenicline (MDW -0.51kg, 95% CI -0.93kg to -0.09kg, n=598). Naltrexone for 6 weeks reduced post-cessation weight gain at the end of treatment (MDW -0.78 kg, 95% CI -1.52 to -0.05 kg; 2 trials, n=179) but the effect was not maintained. Fluoxetine 30mg and 60mg/day combined limited post-cessation weight gain at the end of treatment (MDW -0.99 kg, 95% CI -1.36 to -0.61 kg; 2 trials, n=144).
