Distal subungual onychomycosis is common in toenails. The most common target is the bigtoe nail, maybe because of repeated minor traumas challenging the barrier of the hyponychium. In this case Trichophyton rubrum dermatophyte has invaded multiple nails. The fungus spreads towards the base of the nail in yellowish tracks. Oral terbinafine resulted in complete cure.
