A 36-year-old woman had lower abdominal pain. In vaginal ultrasonography, the uterus is normal, with thin endometrium in the midline, and echos originating from a levonorgestrel-releasing intrauterine device. The urinary bladder is visible behind the fundus (on the right side of the screen). When the probe is moved towards the uterine cervix and tilted to the left, a large, echo-free cyst becomes visible. The cyst was situated deep in the pelvis, and it was rather difficult to detect by bimanual palpation.
