Onychomycosis is much less common in fingernails than in toenails. In fingers it is also often unilateral, as seen here. A dermatophyte, Trichophyton rubrum, invades here the whole thickness of the nail plate, "total dystrophic onychomycosis". With few exceptions, the toenails are also affected, and the degree of affection determines the length of therapy. Oral antifungal agents are necessary.
