Cochrane reviews

  • Early bromocriptine/levodopa combination therapy as a strategy to prevent or delay the onset of motor complications in the treatment of Parkinson's disease may not be better than levodopa therapy alone .
  • Speech and language therapy may increase the loudness of patients' voices in Parkinson's disease .
  • Physiotherapy appears to be effective in Parkinson's disease for a short term. Based on indirect comparisons there appears to be no differences between the different physiotherapy interventions used .
  • There is not enough evidence to assess the effectiveness of occupational therapy in Parkinson's disease . There is insufficient evidence on the efficacy and safety of beta-blockers for tremor in Parkinson’s disease . Paroxetine and venlafaxine may be effective for depression in Parkinson’s disease. However, the evidence is insufficient for the efficacy of other antidepressants .
  • MAO-B inhibitors may have weaker symptomatic effects than levodopa and dopamine agonists in early Parkinson’s disease. They may reduce the rate of motor fluctuations compared with initial levodopa therapy and may have fewer significant adverse effects than the older agonists .
  • Parkinson's disease patients with impaired gait probably benefit from treadmill training .
  • In Parkinson´s disease add-on therapy for levodopa may reduce off-time and required levodopa dose as well as improve overall disability scores on the expense of dyskinesia and other side-effects. Dopamine agonists may provide more effective symptomatic control than catechol-O-methyl transferase inhibitors (COMTI) and monoamine oxidase type B inhibitors (MAOBI) therapy .

Other evidence summaries

  • Clozapine appears to be effective for the treatment of drug-induced psychosis in Parkinson’s disease .
